Peer Connect Program Registration

 

We are so glad you are interested in our Peer Connect Program. Peer Connect is a free, confidential, peer support program that connects anyone touched by chordoma with another person whose experiences with this cancer are similar.

You should consider the Peer Connect Program if you’re interested in talking with someone who has been through a similar cancer experience and can help support you through diagnosis, treatment, and life-after-cancer experience. To participate, you will need to complete and submit this registration form. Once registered, we will match you with a trained Peer Guide who has had a similar cancer experience. Guides will provide a listening ear, serve as a sounding board, and if requested, suggest helpful information and resources. Guides will not serve as health professionals and can not provide medical advice. The connection between you and your assigned Peer Guide may involve only a few phone calls, or it may develop into a longer relationship—whatever is most helpful for you.


Instructions: If you are interested in being matched, please provide the information below to help us to best match you with a Peer Guide from our available volunteers. Every effort will be made to match you with a volunteer with a similar background and experiences. All information you provide is kept confidential and will not be shared without your direct consent. Demographic information is requested to assist us with grant applications to support the program.
