Patient Navigation Service
The Chordoma Foundation Patient Navigation Service is a free and confidential resource that serves the worldwide chordoma community by providing personalized information and support to anyone affected by chordoma, at any stage of their journey.
Our Patient Navigators can help answer your questions, provide information about treatment guidelines, help you find qualified doctors, and connect you with others in the chordoma community who have been through a similar journey.
We can also assist you with finding discounted lodging and travel options, understanding financial assistance programs, and supporting appeals to insurance companies.
Here’s how the Patient Navigation Service works:
Fill out the Request Help form below, email firstname.lastname@example.org, or call (888) 502-6109 to be connected to a Chordoma Foundation Patient Navigator.
A Patient Navigator will be in touch with you within 1-2 business days. Your Navigator will talk with you to learn more about your needs, provide you with information, help answer your questions, and connect you to support and resources. All of the information provided will be personalized to your unique situation. (Please note: Navigators are available Monday through Friday from 8:00 AM to 5:00 PM Eastern Time.)
Your Patient Navigator will be there to assist you throughout your journey with chordoma or as long as you need. You will receive regular check-ins, so your Navigator can stay up-to-date on your situation and continue providing you with support.
Our Patient Navigators are native English speakers but can provide written support in other languages using Google Translate.
