Peer Connect Program Registration

We’re so glad you are interested in our Peer Connect Program.

Peer Connect is a free, confidential, peer support program that connects anyone touched by chordoma with another person whose experiences with chordoma are similar.

You should consider the Peer Connect Program if you’re interested in talking with someone who has been through a similar experience and can help support you through diagnosis, treatment, and survivorship (life after treatment). To participate, you will need to complete and submit this registration form. Once registered, we will match you with a trained Peer Guide whose experiences with chordoma are similar.

Guides will provide a listening ear, serve as a sounding board, and if requested, suggest helpful information and resources. Guides will not serve as health professionals and can not provide medical advice. The connection between you and your Peer Guide may involve only a few phone calls, or it may develop into a longer relationship — whatever is most helpful for you.

Our team of Peer Guides is comprised of members of the chordoma community who volunteer with the Chordoma Foundation to be matched with fellow patients or caregivers who need emotional support along their journey with chordoma.

Request a Peer Guide

If you are interested in being matched, please complete the form below to help us match you with the most appropriate Peer Guide. Every effort will be made to match you with a Guide with a similar background and experiences. All information you provide is kept confidential and will not be shared without your direct consent.
